SW Healthcare and South Worcestershire CCG are leading 6 training sessions for the Care Navigators recently introduced in our 32 member practices.
In your role as a Care Navigator, your main objective will be to enhance patients/carer understanding of the different services available to them.
Focusing on the patients on the Proactive Care Register (mainly frail and elderly patients), you will be able to signpost the patient or their carer to different services depending on their need if they do not need to see a GP.
You will be the point of call for this cohort of patients, as well as the services if they need to contact the practice regarding any follow up needed.
During the monthly Care Navigator meetings, you will be introduced to the teams from each organisation where they will give you more information and details on the services they can offer for your patients. These details then need to be shared with your practice team so that everyone is aware of how patients can access the services.
